Referral Information:
In order to see one of our Cardiologists, many insurance plans require a referral from the primary care physician. It is the patient's responsibility to have the referral completed prior to your first visit. If a referral has not been approved by your insurance plan, you may have to reschedule the visit or become responsible for the charges associated with your visit. Please check with your primary care physician before your visit to make sure they have obtained the required referral from your insurance plan.
